Volunteers needed to help support families or individuals with intellectual or developmental disabilities through their Person Centered Planning (PCP) process, which is the planning process for those who receive services through HealthWest (formerly Community Mental Health) .
Essential parts of the PCP process include: 1) Reviewing the current plan, 2) identifying who you would like to attend your PCP, 3)conducting the pre-plan, 4)attending and participating in the actual PCP, and 5) creating the Plan of Service. The focus will be to align with choice, need, medical necessity and Medicaid eligibility, which does provide for flexibility and community based services; keeping in mind the individual needs and desires of the person served.
The Arc Muskegon will provide training, which includes:
- Mental Health and Medicaid guidelines regarding PCP’s.
- Attending 1 PCP training conducted by an approved list of trainers.
- Advocate manual on best practices.
- Review and acceptance of Arc policies, including confidentiality and conflict of interest.
